Below you will find links to all of the petitions you may need to join us as Kansas Masons, or Shriners. Please download these forms to assist you on your Masonic Journey.

Initiation Petition – Use this petition to apply for membership in Midian Shrine.

Restoration Petition – Use this petition to restore your membership in Midian Shrine if you were suspended or demitted.

Blue Lodge Restoration Petition – Use this petition to restore your membership in your Blue Lodge if you were suspended or demitted.

Affiliation Membership Petition – Use this petition if you were a Shriner in another Shrine Center and want to join the Midian Shriners. NOTE you have to demit from your current Shrine Center in good standing before doing this.

Associate Membership Petition – Use this petition if you belong to and your dues are current in another Shrine Center other than Midian Shrine and would like to apply for a dual membership with Midian Shrine.

Demit Petition – Use this petition to resign your membership in good standing. Note your dues need to be current before demitting.
